 9月のパリの学会の抄録締め切りが3月20日のため、ひとまず申し込みをしました。
 東京女子医大皮膚科教授の檜垣祐子先生が発表いたします。わたしは共同演者です。
 再発型性器ヘルペスのQOLについてのアンケートはすべて宮本町中央診療所の患者さんにご協力をいただきました。
 再発型性器ヘルペスの患者さんは、今度は何時再発するのかという不安、パートナーにうつしはしないかなどと、絶えずストレスと戦っています。今回はその患者さんのQOLについて検討をいたしました。

IMPACT OF RECURRENT GENITAL HERPES ON JAPANESE PATIENTS’ QUALITY OF LIFE

Recurrent genital herpes (RGH) is a common sexually transmitted disease caused by infection with the herpes simplex virus. In many patients, this disease, with its unpredictable nature of recurrences, causes fear of transmission and leads to impaired social and sexual activity.
To quantify the impact of RGH on quality of life in Japanese patients, a cross-sectional questionnaire study was carried out using a battery of instruments: the Japanese version of Skindex-16, WHO-QOL26, and General Health Questionnaire 30 as well as a questionnaire on patients’ history of RGH and their satisfaction with the care they currently receive.
As a result, 54 patients completed the questionnaires. The frequency of recurrences per year was 1 occurrence or less: 13.3%; 2-3 times: 47.2%; 4-5 times: 18.9%; 6 times or more: 20.8%. The current treatment included patient-initiated antiviral therapy: 68.5%; suppressive antiviral therapy:29.6%. The mean scores of the instruments were as follows, Skindex-16-Symptom:36±34; Emotions:45±34; Functioning:22±26, QOL-26-Physical:3.36±0.46; Psychological: 3.25±0.56; Social relationships:3.25±0.68;
Environment:3.32±0.51;General:2.69±0.60,GHQ30:8.6±5.9. Patients with frequent recurrences showed higher scores (lower quality of life) in the Emotion and Functioning sections of Skindex-16 than those with lower frequencies. Patients stated to be satisfied with the current care was 50.0%, while 18.6% were unsatisfied. Patients receiving suppressive antiviral therapy showed better quality of life than those on other treatments.
The present study revealed that RGH affects patients’ health related quality of life especially in aspects of emotional and social functioning. The impact of the disease showed itself more significantly in patients with a higher rate of recurrences.
In conclusion, it can be said that Japanese RGH-patients’ quality of life is highly impaired and effective strategies for its improvement are necessary. The suppressive antiviral therapy, introduced to Japan in 2007, is expected to contribute considerably to an improvement of the quality of life in patients with recurrent genital herpes.
